This year in the month of August and September there has been an increased incidence of Japanese Encephalitis being reported all over Bengal. Approximate 250 deaths occurred during this period.Keeping this public and medical interest in the backdrop We are reporting a case regarding 28 yrs. lady admitted to the ward with acute onset flaccid paralysis of both lower limbs, neck stiffness, diplopia , nasal intonation of voice ,difficulty in swallowing and facial nerve palsy.
The intensivist was called for worsening respiratory failure and the patient was put on mechanical ventilator after shifting to ITU. On neurological examination, power of lower limb was 3/5, jerks were diminished,planter reflex was non-responsive in both lower limbs, gag reflex was absent. The patient was proved to have Japanese encephalitis infection by a four-fold rise in virus-specific antibody detected in paired acute and convalescent sera by enzyme-linked immunosorbant assay. In NCV test she had bilateral symmetrical primary demyelinating secondary axonal type sensory>>motor polyradiculo neuropathy , (upper<
