Comparison of Intrathecal Ketamine and Ketamine Plus Midazolam in Spinal Anaesthesia with Bupivacaine for Orthopaedic Surgery

METHODS :

Fifty patients of ASA grade I and grade II, between 20-50 years of age, weighing between 40-60kg, scheduled to undergo lower limb surgeries were randomly selected. A prospective, randomized study was conducted in following two groups of patients. Each group had 25 patients: GROUP A (n=25) :Patients received 3ml of 0.5% bupivacaine + ketamine (0.1mg/kg) plus normal saline total was made to 3.5 ml. GROUP B (n=25):Patients received 3ml of 0.5% bupivacaine + ketamine (0.1mg/kg) + midazolam(0.02mg/kg), total 3.5 ml.

RESULTS :

The time to onset of sensory block and onset of motor block were significantly higher in group A.The duration of sensory and motor block was significantly greater in group B.The hemodynamic stability was comparable in both groups.The pain-free interval was significantly greater in Group B compared to Group A.

CONCLUSIONS :

A low dose of midazolam and ketamine with bupivacaine intrathecally results in prolonged analgesia and less haemodynamic fluctuations.
