A double blinded randomised controlled trial comparing preoperative single dose oral paracetamol 1000 mg verses placebo for postoperative pain relief in endoscopic urological surgeries

ABSTRACT :

To study the analgesic efficacy of Oral Paracetamol 1000 mg pre operatively for patients undergoing day care endoscopic urological surgeries under spinal anaesthesia for post-operative pain relief.

METHODS :

It’s a randomized, double-blinded, placebo-controlled study. Sixty patients (ASA I and II), were divided into two groups. Group P (Paracetamol) was given 1gm paracetamol and Group C (Control) was given placebo 1 hr before surgery. Post operatively Pain according to NRS and time to rescue analgesia was noted. Statistical analysis was done with Chi square and Mann Whitney U test.

RESULTS :

Patients didn’t complain of pain for the first 21/2 hours in the post-operative period in both the groups. At 3rd hour, none in Paracetamol group complained of pain but 26.7% of patients in the Control group complained of mild to moderate pain. Till 4th hour 87.7% in the Control group complained of pain wherein only 10% in Paracetamol group complained. Till 5th hour, 100% in the Control group vs 43.4% in paracetamol complained of pain. By 6th hour all patients in both the groups complained of pain.

CONCLUSIONS :

Preemptive paracetamol gives better pain relief than placebo for mild to moderate post-operative pain for the initial 4-6 h and further doses of paracetamol if continued postoperatively will give good pain relief after 6 h also. Hence opioids can be avoided and patients can be safely discharged on a day care basis.
