Anesthetic Management Of Temporomandibular Joint Ankylosis – A Review Of 50 Cases At Osmania General Hospital, Hyderabad.

ABSTRACT : Evaluate the concept in preoperative preparation, premedication, induction and intubation techniques, peroperative monitoring and post-operative complications following Temporomandibular joint ankylosis surgeries (TMJ Arthroplasty).

METHODS : Fifty patients belonging to both sexes with TMJ ankylosis from various causes were selected for the study. They were in the age group of 3 years to 45 years. They were all evaluated, investigated and posted for surgery after meticulous pre-operative preparation. The concepts in pre-operative preparation, anesthetic technique and post-operative period will be discussed in detail in the conference.

RESULTS : An immediate outcome in most of the patients following surgeries is quite satisfactory. In a few patients, the mouth opening after surgery was inadequate and the results and reasons for success or failure of surgery will be discussed. In two patients due to micrognathia, Internal Mandibular Fixation had to be done at the end of surgery to stabilize the unstable mandible following release of ankylosis to preserve and maintain a patent upper airway.

CONCLUSIONS : Blind nasal intubation with or without general anesthesia is a very useful and valid technique even in the present times if practiced in a scientific way with a spontaneous breathing technique and regional block analgesia concept.
