Comparative Analysis Of Spinal Vs General Anaesthesia For Gynecologic Laparoscopic Surgeries (Diagnostic Laparoscopy) – A Prospective Observational Study

Objective : Diagnostic laparoscopy has become a well developed surgical procedure and has been routinely employed as a sole technique. This study was undertaken to compareoutcomes of SPINAL ANAESTHESIA and GENERAL ANAESTHESIA for gynaecologic laparoscopic surgeries (diagnostic laparoscopy in infertility) , with respect to the following parameters i.e. Intra operatively : Haemodynamic stability, Quality of analgesia, Oxygen saturation and surgical comfort, Post operatively: Analgesia, Incidence of PONV.
Materials &methods : Prospective observational study of 60 patients with primary infertility were posted for diagnostic laparoscopy, who were randomly divided into two groups, i.e. under spinal (n=30) and general (n=30) anesthesia between the academic year Nov 2013 and Sep 2014. The inclusion criteria of this study were ASA I & II grade patients, BMI <26 with normal coagulation profile. VAS score was used for pain assessment postoperatively. The dose of analgesic required as well as the length of hospital stay was also recorded.
Results : All the procedures were completed by the allocated method of anesthesia as there was no conversion from spinal to general anesthesia. Pain was significantly less for the spinal anesthesia group compared to GA. The total costs was also significantly less in SA group than GA. There was no difference between the two groups regarding complications, hospital stay, recovery or degree of satisfaction at follow up.
Conclusion : We conclude that SA is associated with less anaesthetic cost compared to GA, lower postoperative pain and comparable hospital stay and this form of anaesthesia may be an appropriate choice and can increase the number of patients eligible for surgery.
