The use of a single syringe ketofol (mixture of ketamine and propofol) in direct laryngoscopic examination and biopsy procedure.

Aim:- to evaluate the effect of ketofol on hemodynamic response and recovery parameters and to compare it with Propofol alone in direct laryngoscopic examination/biopsy(DLBx)
Method:- 200 patients posted for DLB were allocated in this prospective double blind study. patients were randomly assigned to either Group KP (Ketamine 50mg+Propofol 100mg in 10cc) or Group P (Propofol 100mg in 10cc). Inj.Succinylcholine 1mg/kg given in all patients.Pulse,BP and Spo2 were noted at 01,3,5,7,10 minutes and after every 10 minute's interval up to discharge. Total volume used in each procedure was noted.

RESULTS : Results:- Time to eye opening was earlier in Group KP (10.56+/-4.22 v/s 14.82+/-2.66 , P value<0.0001) than Group P. Time to follow the verbal command was longer in Group P than Group KP(30.6+/-5.2 v/s 20.2+/-5.5, P value <0.0001). There were no unpleasant psychological reaction following Ketofol. In Group P, marked hypotension (p <0.0001) was noted in 6 patients and Spo2 fall up to 90% was noted in 20 patients.Analgesic requirement in recovery period was high (35 v/s 4) in Group P. Mean dosage of ketofol (23.25+/-0.79mg ketamine , 46.5+/-0.79mg) and Propofol (9.4+/-1.5mg) was required in procedures.
CONCLUSIONS : Conclusion:- Adjuvant use of ketamine with propofol in direct laryngoscopic examination provides significant analgesia , hemodynamic stability and rapid recovery.
