Comparative Study of C Mac D Blade & Macintosh Laryngoscope for Nasotracheal Intubation in Patients undergoing head & neck cancer surgeries.

ABSTRACT : To compare between C Mac D blade and Macintosh laryngoscope in terms of success rate of intubation,time taken to intubate,ease of visualization of glottic by Cormack Lehane grading,use of accessory maneuvers ,hemodynamic changes and occurrence of any truama

METHODS : METHODS : In this single centred, prospective,randomised comparative study,after approval by Institutional Ethics Research Committee , 100 adult patients with anticipated difficult airway (EGRI<7) were intubated after proper randomisation as group 1 (n=50,C Mac D blade) and group 2 (n=50,Macintosh laryngoscope).The primary outcome measured is ultimate success.Secondary outcome measures were time taken to intubate,Cormack Lehane view,need for use of accessory maneuvers, haemodynamic response to laryngoscopy and intubation and occurrence of any trauma

RESULTS : RESULTS : The success rate with C Mac D blade was 100 whereas 20% of patients couldn’t be intubated with Macintosh laryngoscope.Time taken to intubate is shorter with C Mac D blade (median 40 secs). Cormack Lehane grade 1 view was obtained in all cases with C Mac D blade.Use of accessory maneuvers were less in C Mac D blade group. C Mac D blade resulted in less heamodynamic changes than Macintosh laryngoscope.

CONCLUSIONS : CONCLUSION : V ideolaryngoscope seems to be a useful tool in the settings of difficult airway like in head and neck cancer patients.
