Anesthetic implications for management of refractory seizures- WADA test &Hemispherotomy

Anesthetic management for localization of seizure foci and peri operative management of hemispherotomy made us to report this case.

METHODS :

A eight year old child with refractory seizures due to Hemiplegia Hemiconvulsions Epilepsy syndrome, presented with refractory seizures 0f 8-10 episodes per day with weakness of right upper & lower limbs.The patient was started on multiple anti epileptic drugs.(carbamazepine,clobazam,sodium valproate )Without getting any response to medications,surgeons planned for WADA test under monitored anesthesiacare.WADA test revealed that language centre has shifted to right side.Video EEG localized foci as left hemispheric epileptogenic focus.Hence, patient was posted for Left hemispherotomy.

CONCLUSIONS :

HHE syndrome is a unique syndrome that poses unique challenges to anesthesiologist. so a proper plan has to be planned prior to the management. good coordination between neurosurgeon, anesthesiologist, nuerologist is manadatory for the better outcome of the patient.
