Clinical Profile And Outcome Of Methemoglobinemia Due To Self Poisoning, A Case Series.

Methemoglobinemia causes decreased oxygen availability in peripheral tissues and can present as cyanosis or organ dysfunction due to hypoximea. This study Was performed to know clinical profile and outcome of patients with methemoglobinemia due to self-poisoning
METHODS :A retrospective case series of 4 Patients who presented to our hospital with suspected organophosphorus compound poisoning and then clinically diagnosed as methemoglobinemia were included .Their clinical Outcome and laboratory profile Like serum cholinestrase levels were studied noted
RESULTS :There were 3 male & one female pt in the study. One of them presented with cyanosis and ST segment depression in the ECG which became normal after inj methylene blue 1mg/kg infusion. The other 3 patients presented with low GCS score and low oxygen saturation in pulse oximetry and hence intubated for Mechanical ventilation. Two out of four responded well with inj methylene blue 1mg/kg and were discharged after 4 days. The other 2 died after being on ventilator for >7 days , possibly due to treatment delay by 24 hours due to misdiagnosis and late presentation
CONCLUSIONS :Methemoglobinemia due to self poisoning can have a wide presentation.Earlyrecognition and treatment is the key to prevent adverse fatal outcome
