“To Compare Efficacy of Non-Inflatable Cuff of i-gel with Inflatable Cuff of Laryngeal Mask Airway Supreme in Providing an Adequate Seal in Anesthetised

Two new LMA devices with gastric access tubes have been developed: the LMA Supreme and the i-gel. We studied the efficacy of these devices in providing a satisfactory seal during controlled ventilation.
Objectives: To compare the efficacy of LMA Supreme with i-gel with regard to ease of insertion, gastric tube placement, laryngeal seal pressure (LSP) and efficacy during mechanical ventilation. Methodology:42 patients posted for surgeries under GA were divided into 2 groups – LMA-S and i-gel. After induction with fentanyl, propofol and vecuronium, LMA was inserted. Cuff pressures, LSP, leak volume, leak fraction and position of the device were assessed.
Results:The rate of successful insertion in LMA-Supreme group was 95.24% and i-gel Group was 85.71%. In LMA Supreme group, 75% patients had grade I & II fibreoptic view and 25% patients had grade III & IV view. In i-gel group, 61% patients had grade I & II fibreoptic view and 39% patients had grade III & grade IV view. The median LSP was 18.50 cmH2O and was 21.5 cmH2O in LMA-Supreme and i-gel groups respectively.
Conclusion:Both the devices provided an adequate seal for positive pressure ventilation in anaesthetized paralyzed patients. Ease of insertion, gastric tube placement and fiberoptic visualization of glottis were comparable in both group.
