To compare the efficacy ultrasound guided ILIH or TAP block for acute postoperative pain relief following LSCS under spinal anaesthesia.
METHODS :Sixty patients who underwent LSCS under spinal anaesthesia with 1.8ml of 0.5% hyperbaric Bupivacaine were randomly allocated into 2 equal groups to receive bilateral ultrasound guided, TAP or ILIH nerve block with 20ml or 10 ml of 0.25% Bupivacaine respectively. Post operative follow-up included assessment of pain relief during rest and movement, rescue analgesics consumption and complications.
RESULTS :All patients in both the study groups required one dose of Inj. Diclofenac sodium 75 mg intravenously, but subsequently 57% of patients didn’t require any further analgesics till 24 hrs in TAP block group where as in ILIH group only 13% didn’t require further analgesics (p=0.00). The analgesia duration in TAP and ILIH group was 409 ± 206 minutes and 330 ± 189 minutes respectively (P=0.125). None of the patients registered any complications.
CONCLUSIONS :TAP block provided more effective post operative analgesia than ILIH nerve block following Lower segment caesarean section under spinal anaesthesia.
