Comparative study between USG guided abdominal field blocks versus port site infiltration in laparoscopic cholecystectomy for postoperative pain relief.

To study and compare the efficacy of USG guided bilateral posterior rectus sheath block and right subcostal TAP (abdominal field blocks) versus port site infiltrations in patients undergoing laparoscopic cholecystectomy for post operative pain relief in terms of quality, patient satisfaction and compare the total consumption of fentanyl via PCA 24 hours post operatively.
METHODS :Forty ASA 1 and 2 adult patients were randomized to receive abdominal field blocks or Port- site infiltration in addition to standard care including PCA Fentanyl and NSAIDS. The Chi Square test and Proportion are used to compare categorical (Qualitative) variables while Mean and Standard deviation were calculated for quantitative variables. The t –test was carried to compare the numeric variables and Mann Whitney U – Wilcoxon W non parametric test was used to compare ordinal variables between two groups.
RESULTS :There is a highly significance (t=4.87, p<0.01) difference in the total consumption of fentanyl in 24 hours in two groups. Patients receiving abdominal field blocks were highly satisfied (Chi square=15.467,p<0.001) and the mean NRS scores is less than port site infiltration after 2 hours of operation and is significant.
CONCLUSIONS :By this study it is clear that the patients receiving abdominal field blocks are more satisfied, with lower pain scores and reduced fentanyl requirement.
