Comparative Evaluation Of Haemodynamic Response To Conventional Tracheal Extubation And Removal Of Classic Laryngeal Mask Airway (Lma-Classic) Following Bailey Manoeuvre: A Randomized Controlled Study

ABSTRACT : This study is undertaken to evaluate and compare conventional tracheal extubation and removal of classic laryngeal mask airway (LMA-Classic) following Bailey manoeuvre (ETT/LMA exchange technique) for haemodynamic changes and other parameters.
METHODS : A total of 100 American Society of Anaesthesiologist Grade I adult patients undergoing elective surgery under general anaesthesia were selected and randomly divided into two groups of 50 patients each. In Group I (GET) conventional tracheal extubation and in Group II (GCL) LMA removal following Bailey manoeuvre (ETT/LMA exchange) was performed. Haemodynamic parameters were measured before and at 1, 2, 3 ,5 ,10 minutes of tracheal extubation, ETT/LMA exchange and LMA removal.eCoughing/bucking, restlessness, post-operative sorethroat, upper airway obstruction following tracheal extubation or LMA removal for both groups were recorded.
RESULTS : In Group I, there was a significant rise in blood pressure (systolic, diastolic and mean arterial pressure) and heart rate in contrast to insignificant rise in Group II. Incidence of coughing, bucking, restlessness, post operative sore throat and upper airway obstructionis more in GET group as compared to patients in GCL group.
CONCLUSIONS : LMA removal following Bailey manouvre has lesser haemodynamic changes and incidence of coughing, bucking, sorethroat, restlessness and laryngospasm as compared to conventional tracheal extubation.
