Case report - Management of Thoracic injury caused by bull horn.

Introduction : The problem of stray cattle, which had led to many road accidents, including fatalities is quite rampant. Amongst these, deaths caused by Bull and other cattle are quite common in rural India. However, the incidence of such injuries is quite low in urban dwellings. Here we report a case of successful management of a victim who sustained injuries caused due to bull horn.
Case report:Sixty year old male patient was brought to casualty with bull horn injury to left side of the chest and right leg. Emergency resuscitation was done by a group of doctors involving Anaesthesiologists, Cardiothoracic surgeons and Orthopaedicians. Emergency wound exploration and fixation of clavicle fracture was done under general anaesthesia.
Conclusion: Bull horn injuries of thorax are rare, unpredictable and potentially life threatening. Penetrating chest wall injuries may result in sternal dehiscence, flail chest, tension pneumothorax or mediastinum or both and cardiac tamponade. The successful management depends on immediate resuscitation, fast evaluation and therapeutic intervention. Key words: bull horn, resuscitation, general anaesthesia.
