Vecuronium Induced Bronchospasm In A Pregnant Patient

ABSTRACT : CROSS REACTIVITY TO OTHER NEURO MUSCULAR BLOCKING AGENTS IN A VECURONUM SENSITIVE PATIENT

CONCLUSIONS: VECURONIUM INDUCED BRONCHOSPASM IN A PREGNANT PATIENT

A 22 yr old primi was scheduled for emergency caesarean section ,in view of foetal bradycardia. She was suffering from a viral fever and her platelet count was 60,000/ mmз, no evidence petechial haemorrhages. In view of low platelet count General anaesthesia was considered as the plan of anaesthesia , patient was induced with InjThiopentone sodium and airway is secured with rapid sequence intubation with injsuccinyl choline 100 mg. 2 min after injVecuronium 4mg, peak airway pressures were increased and Sp02 falled to 90%, all other causes for tube kinking were excluded out and the depth of anaesthesia is increased and inj hydrocortisone 100mg is injected , peak airway pressures gradually decreased and the SP02 graduallyincreased.after 20 min maintainance dose of vecuronium 1mg was given and the patients peak air way pressures increased and spo2 felled again , and the patient responded to the inj adrenaline 0.1 mg subcutaneously, considered it was bronchospasm induced by vecuronium and the rest of surgery is carried without muscle relaxation and with adequate depth of anaesthesia
