Renal Transplantation-Anaesthetic Experience Of 106 Cases.

ABSTRACT : To analyse anaesthesia management strategies, outcome, and survival of grafts in renal transplantation,both living related and cadaveric, conducted at our centre over a period of 10 years
METHODS : In this retrospective observational, single centre based study, we reviewed medical records of 106 renal transplant recepients, conducted from January 2005 -May 2014 at Osmania General Hospital, Hyderabad. Areas of our interest include patients\' demographic data, preoperative status and optimization, Anaesthesia management (GA+ Epidural analgesia), intraoperative hemodynamic stability, fluid management, perioperative complications, postoperative care and outcome.
RESULTS : Our study included 106 renal transplant recepients who were in the age bracket of 15-55 years, with median age being 25-35 years.Of the total number of patients 4(3.8%) underwent cadaveric renal transplant while the rest 96.2% underwent living related transplant. Predominantly male patients 94(88.7%) and 12 females(11.3%).The primary cause of renal failure was glomerular in 47.5%, tubulointerstitial in 47.5% and other causes in 5%. Anaesthesia technique was standardised for all patients. Epidural analgesia effective in 80%, difficulty by technique in 9% and contraindicated in 11%. Quality of life was good with stable graft function in 85.5%, whereas 14.5% patients required hemodialysis
CONCLUSIONS : Proper patient selection, preoperative optimization, effective anaesthesia management strategies with emphasis on intraoperative hemodynamic stability maintenance and good postoperative care resulted in good quality of life in 85.5% of renal transplant recepients at our centre
